Service Model

We currently provide extensive skilled nursing and home health aide services to clients in large assisted-living facilities as well as individual care and supervision of residents at a new senior residence in Lakeville called Greystone Manor.

We staff facilities and senior residences 24 hours a day, 7 days a week, every day of the year with on-site Home Health Aides (HHAs) under the periodic on-site supervision of a Registered Nurse (RN). The RN is on-call 24/7. On-site staff respond to medical emergencies and provide immediate assistance as appropriate including calls to 9-1-1 for paramedics, calls to the resident's personal physician and / or calls to a family member designated for notification.

Registered Nurse Case Management

  • Maintain basic health information to facilitate appropriate care
  • Conduct over-all management and monitoring
  • Perform health assessment every 60 days
  • Monitor and record blood pressure on a weekly basis
  • Provide counseling on health-related problems
  • Promote physician follow-up of health problems
  • Advise residents about their disease process and medications

Additional Services

Supplemental skilled nursing care, home health aides, personal care, and companion services are available for extended hourly care requiring continuous one-on-one care of four or more hours per day. When more than one staff member is required to provide a service, charges increase appropriately.
